    Coordinates, plans and implements services for My Home Housing Program participants.

    • Performs assessments and creates service plans with clients
    • Performs home visits with each client at least two times per month
    • Assists mobility-impaired clients with shopping, errands, and transportation to critical medical appointments
    • Assists clients with coordinating relocation to new rental units
    • Communicates and meets with clients’ collateral contacts
    • Guides/coaches clients with budgeting and money management
    • Refers clients to appropriate community resources and enrichment opportunities
    • Coaches clients with job seeking and resume writing
    • Provides assistance to clients who are applying for SSI/SSDI and other benefits
    • Develops and maintains therapeutic relationships with clients
    • Ensures clients are following program rules and regulations
    • Documents every contact with clients in the clients’ files
    • Completes paperwork to The Salvation Army and Milwaukee County Special Needs Housing Programs requirements
    • Gathers and maintains necessary client paperwork and documentation 
    • Completes referral and exit paperwork for clients
    • Coordinates and delivers needed donations to clients  
    • Maintains relationships with landlords and serves as a liaison to the landlords regarding clients’ property needs



    The requirements listed below are representative of the minimal education, experience, knowledge, skills, and/or abilities required for this position. 


    Education:   Bachelor’s Degree in Social Work, Human Services or a related field


    Certifications: Wisconsin State Social Work license; valid Wisconsin Driver’s license with clearance to drive from The Salvation Army’s insurance carrier.


    Experience:  Two years case work experience in a social service agency, or fieldwork.




    Knowledge of:

    • Principles and practices of social work and case management techniques relevant to the services offered.
    • Professional boundaries and ethics for licensed social workers.
    • Public and private social services providers within the community and community resources and programs available to clients with identified needs.
    • Collaborative case planning.
    • De-escalation and crisis intervention techniques.
    • Computer proficiency, including a working knowledge of Microsoft Office programs and use of email
    • Appropriate business and record keeping principles and practices.
    • Good interpersonal skills. Techniques for dealing with a variety of individuals from various socio-economic, ethnic and cultural backgrounds, in person and over the telephone.
    • The Salvation Army’s Safe From Harm program.

    Ability to:

    • Assess cases appropriately and utilize the most appropriate community resources to provide effective client services.
    • Prepare clear and concise reports, correspondence and other documentation.
    • Use initiative and independent judgment within established procedural guidelines; organize own work, set priorities and meet critical deadlines.
    • Establish and maintain effective working relationships with those contacted in the course of work.
    • Communicate effectively: orally, in writing and over the telephone.
    • Plan, implement and follow through with projects relating to the position
    • Respond to after-hours calls and emergencies as appropriate.

The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by the job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.  Employee must have reliable transportation to move about service area and between Salvation Army locations.


    Travel: Regular travel throughout the community to serve clients and perform home visits. Out of town and/or overnight travel to conferences and training may be required, but rare.

    Much of this job is performed indoors in normal room temperature conditions.  When the duties require being outdoors, the employee is exposed to weather conditions prevalent at the time. The noise level in the work environment is usually low to moderate.

    At times the duties will involve working with individuals having infectious and/or communicable diseases.
