  • Overview

    The incumbent is primarily responsible for assisting with daily program activities, charting on participant involvement in activities and assisting with ADL’s for participants in the Adult Day Center.


    General Duties

    Assist Activities Coordinator in setting up for, implementing, charting on participant involvement and coordinating daily activities for participants, including special events.
    1. Provide assistance with check-in and sign-out (bussing) of participants as assigned.
    2. Read the daily communication book and initial.
    3. Assist with daily up-keep of adult day center areas including minor clean-up following snacks, lunch, and end of day clean-up.
    4. Lead participants in exercise time or afternoon stretch time as assigned (once trained).
    5. Chart on participants progress or involvement in activities as assigned.

    Dietary Duties

    Perform any kitchen assignments or fill-in for Dietary Staff as assigned.
    1. Prepare and serve snacks; mornings if working in kitchen, prepare and set up afternoon snack with beverages as assigned in pm.
    2. Assist with set up for lunch and assist participants during the meal.
    3. Clear plates and place in dishpans for dietary staff to wash.
    4. If in kitchen, rinse plates, glasses, coffee cups and silverware and run through dishwasher to sanitize.
    5. Perform above duties if assigned for pm snack.
    6. Wipe and sanitize all kitchen counters and surfaces following use.
    7. Put away all dishes, cups, glasses, and silverware following sanitizing.

    Restroom Duties

    Direct and assist participants in the restroom as needed.
    1. Accompany participant to restroom.
    2. Assist participant(s) on restroom sheet with predetermined needs such as replacement of sanitary pads or briefs, removal and replacement of clothing, clean up when soiled and locating of restroom.
    3. Inform janitor of any issues with restrooms needing replacement of paper products or any repair or necessary clean up to prevent injury to participants.
    4. Report any skin integrity issues, bruising or other health related issues to the RN or designee.
    5. Trash Cans – report need to empty to janitor if excessively full.
    6. Bathroom Floor - should be free of garbage, water or soiling to prevent falls with participants.
    Report issues as such to a janitor or in their absence you may need to sweep or mop the floor.
    7. Lights - all lights should be in good working order. If a light is out please inform a janitor of need to replace.

    Community Room or Dining Room

    1. Assist participants as necessary to maneuver to chairs or tables, using transfer belt as needed.
    2. Keep chairs, walkers, wheelchairs and any other apparatus out of pathways where participants will travel.
    3. Wipe tables (and chairs as necessary) following crafts or artwork.
    4. Set up room for program usage.
    5. Report any damaged or broken equipment or furniture to Director or designee.
    6. Put tables and chairs in orderly fashion.
    7. Put away all equipment, i.e., hand trucks, storage bins, etc.


    1. Document individual participant involvement on activity tracking sheets either following each activity or at least by end of the day.
    2. Complete any activity assessments or inventories asked to complete for individual participants.
    3. Read and sign off on any communication logs or communications, or progress notes for participants as needed.

    Hallways and Entrances

    1. Keep each walkway from the front entrance to the back free of items left in accordance with fire codes.


    1. Support the mission of The Salvation Army by treating every client and colleague with individual

    2. And all other duties as assigned.


    A High School Diploma or GED equivalent is required; activities experience is preferred. Must be able to follow instructions, regularly lift 50 pounds and transfer 150 pounds and pass The Salvation Army’s background check. It must be understood by the incumbent that the provisions of Safe From Harm will apply upon commencement of employment and also understood that the required Safe From Harm training be satisfactorily completed within the first thirty (30) days of employment.

    1. Must be certified in CPR.
    2. Must have experience in caring for individuals with mild to moderate disabilities.
    3. Must relate well to individuals or adults with disabilities.
    4. Must have strong teamwork skills.
    5. Must be in good physical and mental health and be able to communicate observations of participants to appropriate department heads.
    6. Must be able to pass Department of Human Services Background Study and The Salvation Army Background Study.
    7. Must be able to work closely with all other staff, maintain positive work atmosphere by behaving and communicating in a manner that fosters relationships with co-workers and supervisors. This would include but not be limited to such actions: resolution of conflicts in a professional manner; courteous treatment of staff, visitors and clients; respect of others’ property and person, professional and appropriate communication to and about co-workers, supervisors, and subordinates.
    8. Must attend 8 hours of In-Service Training annually as approved by Director.
