Overview

Provide Case Management to homeless families using Emergency Family Shelter.

Responsibilities

  • Extend case management services to families using Emergency Shelter. 
  • Meet with assigned families weekly, create case plans, track progress and re-evaluate goals as needed. 
  • Have a working knowledge of community resources and make appropriate referrals. 
  • Provide crisis intervention. 
  • Maintain accurate records, files statistics and reports, including Service Point entry. 
  • Network with other service providers. 
  • Maintain positive working relationships with service providers, clients and fellow employees.
  • Other duties as assigned by supervisor or Dane County Coordinator

Qualifications

The requirements listed below are representative of the minimal education, experience, knowledge, skills, and/or abilities required for this position. 

 

Education: Bachelor's degree in social work or related field. 

 

Certifications: Valid Wisconsin driver's license and clearance to drive by The Salvation Army insurance carrier. 

 

Experience: Case and crisis management training and experience. 

 

Skills: Good oral, written and interpersonal communication skills. Exceptional understanding of professional boundaries/ethics.
